If you’re a new client, kindly complete and sign both the intake form as well as the informed consent form prior to your first session.

Intake Form

Please use all the space you need for each answer adding pages if necessary and ensure you complete all the applicable questions even if they are repetitious. This form will take you 30 to 40 minutes to introspect and answer.

Informed Consent Form

Please print both forms (using any popular PDF reader, like Adobe Reader) filled out by hand and sign at the bottom. Kindly bring them in for your first session. Failure to complete the forms will result in your having to complete them upon arrival.
